Scheduling an Appointment

-Appointments can only be scheduled via phone call during our clinic's normally scheduled hours.

-Individuals over the age of 18 must schedule their own visits (parents and/or spouses will be unable to schedule for another adult).

​

-Guardians (healthcare or otherwise) must present all legal documents at the first visit.

​

-A legal guardian must be present at the first visit for all minors and patients with intellectual disabilities.

​

-Parents and/or spouses will not be allowed into treatment areas with patients over the age of 5 unless asked by a staff member.
